Campaigners argue that existing eligibility rules leave some carers paying for medication despite the impact caring responsibilities can have on their finances and health.

Prescriptions currently cost £9.90 per item in England, although several groups already qualify for exemptions. The proposed expansion could affect a substantial number of people, with figures from 2021 identifying 5.8 million unpaid carers. Alongside wider eligibility, advocates are calling for better information to ensure people receive support they are already entitled to.

Unpaid Carers Face Gaps in Prescription Support

According to Carers UK, there were 5.8 million unpaid carers in 2021. The figure has been highlighted by campaigners seeking to extend prescription exemptions to people providing unpaid care, although some already qualify through existing arrangements.

Helen Lord, chief executive of the Vulnerability Registration Service, said caring responsibilities can affect employment, household finances and personal wellbeing. Some carers reduce their working hours or leave employment entirely while taking on additional costs.

“Unpaid care can have a profound impact on someone’s finances, employment, physical health and mental wellbeing,” Lord said, according to ChronicleLive.

She also warned that unpaid carers may struggle to look after their own health because of competing responsibilities. While Carer’s Allowance provides £86.45 a week, many people providing care do not qualify for the payment.

Existing prescription exemptions can provide some protection. People aged 60 and over are entitled to free NHS prescriptions, while certain benefit recipients and people with qualifying medical conditions may also receive medication without charge.

Lord argued that caring for someone should not be taken as evidence of financial security. Prescription charges can become another expense for carers already managing changes to their income and daily responsibilities.

ℹ️ NHS App – How To Guide – Requesting a Repeat Prescription

Not sure how to order repeat prescriptions?
Check out this help video : https://t.co/wXUjLYnlwo

📺 Alternatively, see the whole range of help videos here: https://t.co/UMv7Z2m6T2 pic.twitter.com/B5wCr3wsgl
— Middlewood Partnership (@MiddlewoodGroup) October 25, 2026

NHS Outlines Existing Exemptions and Measures to Improve Access

According to the NHS Business Services Authority, eligibility for free prescriptions depends on factors including age, qualifying benefits and exemption certificates. People under 16, those aged 16 to 18 in full-time education and adults aged 60 or over are among those entitled.

Universal Credit recipients may qualify depending on their take-home pay during their latest assessment period. People receiving Pension Credit Guarantee Credit also qualify, whereas receiving Pension Credit Savings Credit alone does not automatically provide exemption.

Pregnant women and those who have had a baby within the previous 12 months can obtain free prescriptions with a valid maternity exemption certificate. People with certain medical conditions must hold a valid medical exemption certificate.

The NHS also operates a Real Time Exemption Checking system at pharmacies, allowing staff to check whether patients qualify when collecting medication. The system can verify qualifying benefit entitlements against Department for Work and Pensions records. According to the Daily Express, Lord wants vulnerable people to receive more proactive information rather than having to identify available assistance themselves.

For those who remain ineligible, prescription prepayment certificates provide another option. A three-month certificate costs £32.05, while a 12-month certificate costs £114.50, covering unlimited eligible prescriptions during the relevant period.

NHS prescriptions remain free in Scotland, Wales and Northern Ireland. In England, the debate centres on whether existing exemptions and information services adequately reach people facing financial and caring pressures.

Millions of unpaid carers could benefit from changes to free NHS prescriptions in England.
