New draft guidance for treating obsessive-compulsive disorder (OCD) in England has raised concerns among psychiatrists who fear it could make specialist treatment harder to access for patients who do not respond to initial care.

Psychiatrists Raise Concerns Over the New NHS Guidance

The proposed changes come as the National Health Service (NHS) prepares to update its approach to treating obsessive-compulsive disorder, a mental health condition affecting approximately 700,000 people across England.

The existing guidance is more than two decades old, making a review of treatment recommendations long overdue.

Yet the proposed update has prompted concern among mental health specialists. According to Channel 4 News, psychiatrists fear the draft could make it more difficult for patients to receive specialist care when their first treatments fail.

Their concern centers on what happens after standard treatment proves insufficient. Patients with persistent symptoms may require more specialized assessment and treatment, particularly when their condition continues to interfere with everyday life.

The guidance remains in draft form, and the concerns reported by psychiatrists do not establish that access to specialist services has already changed.

Why OCD Patients May Need More Than Initial Treatment

OCD involves recurring, unwanted thoughts, images, or urges, known as obsessions, alongside repetitive behaviors or mental rituals, known as compulsions.

These symptoms can consume substantial amounts of time and disrupt work, education, relationships, and daily routines.

People with OCD may recognize that their fears or repetitive behaviors are excessive but still struggle to control them. The condition can cause significant distress, especially when attempts to resist compulsions intensify anxiety.

Established treatments include cognitive behavioral therapy (CBT), particularly exposure and response prevention (ERP), and medications such as selective serotonin reuptake inhibitors (SSRIs).

Many patients benefit from these approaches, but not everyone experiences sufficient improvement.

For people whose symptoms persist, specialist services can offer further assessment and treatment planning. Such care may be particularly relevant when OCD is severe, complicated by other mental health conditions, or resistant to previous interventions.

The psychiatrists’ warning therefore concerns a specific group: patients who need additional help after initial treatment has not delivered adequate relief.

A Long-Awaited Update Raises Questions About Access

Updating clinical guidance after more than 20 years offers an opportunity to reflect developments in the understanding and treatment of OCD.

Clinical recommendations also influence how healthcare professionals identify patients’ needs, select treatments, and determine when more specialized support may be appropriate.

The debate surrounding the draft highlights the difference between recommending an initial course of treatment and ensuring that people who do not improve can access further care.

The available reporting does not establish which specific provisions of the draft might affect referrals, or whether the final guidance will retain them. It also does not demonstrate that specialist services will be withdrawn.

Those distinctions matter because draft clinical recommendations are not the same as implemented changes to NHS services.

For patients whose symptoms remain severe despite treatment, the availability of appropriate follow-up care can shape their options for managing the condition.

The psychiatrists interviewed by Channel 4 News have raised their concerns while England’s OCD treatment guidance is being revised for the first time in more than 20 years.
