According to the MoneySavingExpert website founded by Martin Lewis, recent analysis indicates that close to 900,000 people may be overlooking the opportunity.

The advice centres on the NHS prescription prepayment certificate, a scheme set up to help patients in England manage the cost of repeat medication. With each prescribed item carrying a charge, the savings can add up quickly for anyone collecting several items at a time or returning to the pharmacy month after month.

How the Prepayment Scheme Reduces Costs

A prescription costs £9.90 per item in England, and patients sometimes find themselves paying for several items at once. A prepayment certificate, often described as a prepaid card, is presented to the pharmacist when medicines are collected and removes the need to pay item by item during the period it covers.

Martin Lewis, who founded MoneySavingExpert.com, said the message needed to reach anyone in England who regularly gets prescriptions, including some people living with chronic illnesses. He advised acting sooner rather than later, on the basis that buying now means paying less for longer.

His rule of thumb is straightforward. According to Lewis, anyone averaging more than one prescription a month will usually find a certificate the cheapest option, because it caps the total cost. The certificates provide unlimited prescriptions over either three months or a year. For someone using an average of two prescriptions a month, he said the saving would amount to £120, while also removing the worry of having to pay more if extra medication were needed.

The scale of the missed savings is set out in NHS figures. According to the NHS, around 872,000 patients could have saved roughly £35 million between them in 2023/24 by buying a 12-month certificate, an average of £41 per person.

Two certificates are available, and once purchased each covers all prescriptions during the relevant period. Costs are unchanged for 2026. A three-month certificate is priced at £32.05 and a 12-month certificate at £114.50. There is also a separate certificate for hormone replacement therapy, with a 12-month HRT certificate costing £19.80. By way of comparison, someone collecting two prescriptions a month over a year would save more than £100 against the cost of paying for each prescription separately.

Applications can be made through the NHS Prescriptions website, with payment by card or by direct debit for those who would rather spread the expense. Forms are also available at selected pharmacies, and applications can be made by telephone on 0300 330 1341.

Patients with long-term conditions could save money on their prescriptions.

NHS staff, remind patients to check their eligibility for a Prescription Prepayment Certificate at https://t.co/ZQMCWII8cM. pic.twitter.com/JT5ZxyLa6f
— NHS England (@NHSEngland) April 3, 2024

Who Is Already Entitled to Free Prescriptions

Before applying for a certificate, the advice is to confirm whether you already qualify for free prescriptions, in which case there would be no need to pay anything.

Prescriptions are free for all residents of Scotland, Wales and Northern Ireland. In England, they are free to anyone under 16, anyone aged 60 or over, and those aged 16 to 18 who are in full-time education. Patients with certain medical conditions or disabilities may also qualify, as can people on a low income or in receipt of specific benefits.

For those who do need a certificate, there are several routes to apply. The simplest is online through the NHS service. Applications can also be made by post using the FP95 form, which can be downloaded or collected from a pharmacy or doctor’s surgery. Completed forms should be sent to PPC, NHS Business Services Authority, 152 Pilgrim Street, Newcastle Upon Tyne, NE1 6SN.

Payment by post can be made by cheque or postal order, and a direct debit can be set up for the 12-month certificate. Applications can be completed over the phone on 0300 330 1341, with a bank card needed to arrange payment. Patients who prefer to apply in person can take the FP95 form to a participating pharmacy, although those choosing this route pay at the till and cannot opt for the 12-month direct debit.

Martin Lewis is urging people who regularly collect medication to check whether a prepayment certificate would cap their costs.
